To prepare for your first visit at Advanced Hearing Aids, please complete the patient forms below and bring them to your appointment. Please download the form, then print it out to complete it and bring it to your appointment. You’re also welcome to complete this form in-office by simply arriving 15 minutes before your scheduled appointment time.
